JANUMET XR (JAN-you-met) is a prescription drug that contains 2 prescription diabetes medicines, sitagliptin (JANUVIA®) and extended-release metformin. JANUMET XR can be used along with diet and exercise to lower blood sugar in adults with type 2 diabetes.

This drug targets 3 main factors to help lower blood sugar levels:

  1. Helps your pancreas make more insulin
  2. Helps decrease excess sugar that your liver makes
  3. Helps your body use the insulin that it makes more effectively


  1. Helps control blood sugar throughout the day, including after meals
  2. Is not likely to cause weight gain and low blood sugar (hypoglycemia) by itself

Type 2 diabetes mellitus
Type 2 diabetes mellitus occurs when your body does not make enough insulin and the insulin that your body produces does not work as well as it should. Your body can also make too much sugar. If this happens, sugar (glucose) levels build up in the blood which result in high blood sugar (hyperglycaemia). This could lead to serious medical issues.

The main goal of treating diabetes is lowering your blood sugar to a normal level. Lowering and controlling blood sugar may help prevent or delay complications of diabetes, such as heart disease, kidney disease, blindness and foot amputation.

What are the ingredients in JANUMET XR?

Active ingredients:

sitagliptin and metformin hydrochloride extended-release

Inactive ingredients:

All doses contain: povidone, hypromellose, colloidal silicon dioxide, sodium stearyl fumarate, propyl gallate, polyethylene glycol, and kaolin. Film coating contains hypromellose, hydroxypropyl cellulose, titanium dioxide, FD&C #2/Indigo Carmine Aluminum Lake and carnauba wax.

In addition the JANUMET XR 50 mg/1000 mg Tablets also contain: yellow iron oxide.

How does this medication work?

This medication contains two active ingredients, sitagliptin (as phosphate monohydrate) and metformin hydrochloride.

Sitagliptin belongs to a class of medicines called DPP-4 inhibitors (dipeptidyl peptidase-4 inhibitors), and metformin belongs to a class of medicines called biguanides.

Sitagliptin and metformin work together to control blood sugar levels in patients with type 2 diabetes mellitus. This means Janumet XR:

  1. helps to increase insulin levels after a meal.
  2. lowers the amount of sugar made by the body.
  3. works when blood sugar is high, especially after a meal. This is when the body needs the greatest amount of help in lowering blood sugar.
  4. also lowers blood sugar between meals.

How to take

Take only as and when prescribed by your doctor.

Your doctor will tell you how many JANUMET XR tablets to take. They should be taken once a day.

Take them with meals to lower your chance of an upset stomach.

Swallow the tablets whole with a glass of water. Do not chew, cut, or crush the tablets.

Tell your doctor if you cannot swallow these whole. You may see something that looks like the JANUMET XR tablet in your stool (bowel movement).

If you see tablets in your stool several times, talk to your doctor.

Do not stop taking this medication without talking to your doctor.

Follow all directions given to you by your doctor and pharmacist carefully.

If you do not understand the instructions on the box, ask your doctor or pharmacist for help.

How long should you take this medication?

You should continue to take JANUMET XR as long as your doctor prescribes it so you can continue to help control your blood sugar.

If you miss a dose 

If you forget to take your meds or miss a dose, take it as soon as you remember. If you do not remember until it is time for your next dose, skip the missed dose and go back to your regular schedule.

If you are not sure whether to skip the dose, talk to your doctor or pharmacist.

Do not take a double dose to make up for the dose that you missed.

If you have trouble remembering to take your tablet, ask your pharmacist for some hints.

How should I store this medication?

Store at 68°F to 77°F (20°C to 25°C). Store in a dry place and keep cap tightly closed. Keep out of the reach of children.

The only difference between Janumet XR vs Janumet is that JANUMET is taken twice daily, while JANUMET XR is taken once daily.

What should I tell my doctor before taking this medication?

Before you take, tell your doctor if you: have or have had inflammation of your pancreas (pancreatitis).
-have severe kidney problems.
-have liver problems.
-have heart problems, including congestive heart failure.
-drink alcohol very often, or drink a lot of alcohol in short-term “binge” drinking.
-are going to get an injection of dye or contrast agents for an x-ray procedure;

JANUMET XR may need to be stopped for a short time.
-have any other medical conditions.
-are pregnant or plan to become pregnant.

It is not known if JANUMET XR will harm your unborn baby. If you are pregnant, talk with your doctor about the best way to control your blood sugar while you are pregnant.

Pregnancy Registry: If you take JANUMET XR at any time during your pregnancy, talk with your doctor about how you can join the JANUMET XR pregnancy registry. The purpose of this registry is to collect information about the health of you and your baby. You can enroll in this registry by calling 1-800-986-8999.

-are breast-feeding or plan to breast-feed. It is not known if this medication will pass into your breast milk. Talk with your doctor about the best way to feed your baby if you are taking this medication.

Tell your doctor about all the medicines you take, including prescription and over-the-counter medicines, vitamins, and herbal supplements. This drug may affect how well other drugs work and some drugs can affect how well the medication works. Know the medicines you take. Keep a list of your medicines and show it to your doctor and pharmacist when you get a new medicine.

Talk to your doctor about when you should stop this medication and when you should start JANUMET XR again.

What are the possible Janumet XR side effects?

Serious side effects have happened in people taking this medication or the individual medicines in JANUMET XR.

-Low blood sugar (hypoglycemia). If you take this medication with another medicine that can cause low blood sugar, such as a sulfonylurea or insulin, your risk of getting low blood sugar is higher. The dose of your sulfonylurea medicine or insulin may need to be lowered while you use JANUMET XR.

Signs and symptoms of low blood sugar may include:

headache, drowsiness, irritability, hunger, dizziness, confusion, sweating, feeling jittery, weaknes, fast heart beat

Serious allergic reactions.

If you have any symptoms of a serious allergic reaction, stop taking this medication and call your doctor right away. Your doctor may give you a medicine for your allergic reaction and prescribe a different medicine for your diabetes.

-Kidney problems, sometimes requiring dialysis.
-Joint pain. Some people who take medicines called DPP-4 inhibitors, one of the medicines in JANUMET XR, may develop joint pain that can be severe. Call your doctor if you have severe joint pain.
- Skin reaction. Some people who take medicines called DPP-4 inhibitors, one of the medicines in JANUMET XR, may develop a skin reaction called bullous pemphigoid that can require treatment in a hospital. Tell your doctor right away if you develop blisters or the breakdown of the outer layer of your skin (erosion). Your doctor may tell you to stop taking JANUMET XR.

The most common side effects of this medication include:
-stuffy or runny nose and sore throat
-upper respiratory infection
-low blood sugar (hypoglycemia) when used in combination with certain medications, such as a sulfonylurea or insulin.
-gas, upset stomach, indigestion
-nausea and vomiting

Taking JANUMET XR with meals can help lessen the common stomach side effects of metformin that usually happen at the beginning of treatment. If you have unusual or sudden stomach problems, talk with your doctor. Stomach problems that start later during treatment may be a sign of something more serious.

JANUMET XR may have other side effects, including swelling of the hands or legs. Swelling of the hands and legs can happen if you take it in combination with rosiglitazone (Avandia® ). Rosiglitazone is another type of diabetes medicine.
